His first match point was 5–4 in the fourth set and then he converted his second to break Grenolers' serve in the final game.
"That ball falling for a long time at the last point was the best feeling of my life," Mektic said. "Today was again an incredible match. The last three matches we played were probably the best three matches of my life, the best three matches I've ever played in."
The pair had to go down two sets in the quarter-finals.
It is the eighth title for Mektic and Pavic in their first year as a pair this season.
They are the first Croatian pair to win doubles at Wimbledon and 20 years after Goran Ivancevic won the men's singles final.
"It's something special, it's the best tournament the world has ever seen," Pavic said. "Especially for me and I think for Nicola as well, we all grew up with Goran."
Pavic also won the Australian Open in 2018 and the US Open last year with different partners. It was Mektic's first Grand Slam title.
They enter the Tokyo Olympics as the big favorites for the gold medal.
"We are very confident," Pavik said. "The schedule is not the best, but I think we have a good chance. We are one of the favorites out there. The way we played, I think we will have a good chance."
